• The FREE DENTAL CHECKUP begins on 01 FEBRUARY 2026 and ends on 31 DECEMBER 2026. 

MECHANICS AND PARTICIPANTS:

1. ACCESS AND ANSWER the oral health questionnaire via Colgate QR Code found on Colgate Maximum Cavity Protection packs.

   WHO CAN ANSWER THE ORAL HEALTH QUESTIONNAIRE:

   • Residents of the Philippines who are at least eighteen (18) years old on or before the start of the Period (01 FEBRUARY 2026) are qualified to answer the oral health questionnaire.

   • Participants may answer the questionnaire on behalf of their child who is at least thirteen (13) years old.

2. OPT IN to provide the following information:

   • Name 

   • Postal Code 

   • Email address 

3. RECEIVE your oral health score and browse through the list of Colgate partner dental clinics affiliated with the Philippine Dental Association.

4. CONTACT your preferred dental to book an appointment for a free dental checkup.

   WHO CAN AVAIL OF THE FREE DENTAL CHECKUP:

   • The participant who answered the oral health questionnaire (at least 18 y.o), or

   • Their child for whom they completed the questionnaire for (at least 13 y.o.);

   • The participant may also transfer the free dental checkup to any person of any age within the first degree of consanguinity 

   • The schedule of appointment is subject to the selected dentist’s availability.

COVERAGE:

• The “Free dental checkup” shall be limited to basic dental check up. It is not inclusive of oral prophylaxis or any other procedure or service. Any cost for any additional procedure or service provided by the dentist and/or requested by the participant shall be for the account of the participant.

OTHER TERMS AND CONDITIONS:

• All standard mobile text, call and data charges for the participant’s coordination with the dental clinic will apply and shall not be shouldered by Colgate-Palmolive Philippines, Inc.

• Any person who commits, conspires to commit or in any way aids in the commission of any act of tampering, counterfeiting, or fraud shall be automatically disqualified from this promotion and be subject to legal action under Philippine laws and regulations.

• Availment of the Free Dental Checkup is subject to force majeure conditions. Colgate-Palmolive Philippines, Inc. reserves its right to change, discontinue, extend, or modify the terms and conditions thereof, subject only to notice to and/or approval by the FDA.

• Colgate-Palmolive Philippines, Inc., its officers, employees, personnel, subsidiaries, affiliates,  and third party agencies assume no responsibility or liability for any of the acts and/or omissions of the assigned dentists in relation to the services provided by them to the availers of the free dental checkup.

• Privacy Notice. Colgate-Palmolive Philippines, Inc. (“CPPI”) is committed to respecting your privacy and upholding your rights as owners of personal information, pursuant to applicable data privacy laws and Corporate policies. By participating in this activity, you understand that the collection and processing of your personal data (name, address, and contact details) in relation to this Promo will be for the purpose of processing your availment of the free dental check-up.

  The dentists servicing you are considered independent personal information collectors and processors and CPPI shall not be responsible for the processing and collecting of your personal information for purposes of providing you dental services.

  Data transfers, outsourcing. You understand that collection and/or processing may be done directly by CPPI, or shared for processing to its affiliates, partners, agencies, and content service providers.

  Retention and disposal. Your personal data shall be stored and accessed only for the duration necessary to implement the promo, unless otherwise expressly permitted by you or unless required to be retained by law.

  Data subject rights. All personal data processing activities shall be undertaken in compliance with the Data Privacy Act of 2012 and all applicable laws.
